چکیده

The fate is described of 11 patients who had a stored pulmonary valve homograft mounted on a frame or in a Dacron jacket used for mitral valve replacement. Mitral regurgitation due to holes in the attenuated cusps occurred in nine by one year, requiring re-operation or causing death. Only one patient remains well with moderate mitral regurgitation after two years. Since January 1967, 118 patients have had mitral valve replacement with a preserved cadaver semilunar homograft valve mounted in a jacket or frame (Murray, Roschlau, and Lougheed, 1956; Heimbecker et al., 1962). In all except 11 patients the aortic valve was used. However, in the 11 patients in whom an inverted pulmonary valve was used (Lower, Stofer, and Shumway, 1961) the clinical course has been different. The purpose of this report is to draw attention to the problems and discourage the use of this form of mitral valve replacement.

Right sided heart evaluation after successful mitral valve replacement.

Introduction: It is well-documented that right-sided heart dysfunction and significant tricuspid valve regurgitation (TVR) have adverse effects on patient outcomes after left-sided heart valve surgery. Therefore, the evaluation of right ventriclular (RV) function and TR severity in patients who had undergone mitral valve replacement (MVR), associated with/without concomitant su...

High-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient

Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...

Clinical and Hemodynamic Studies in Patients

Debate continues regarding the long-term clinical and hemodynamic benefit of homograft replacement of the diseased mitral valve. The results of valve replacement with mitral homografts in the 120 patients who have had operation at the Stanford Medical Center from May 1967 to November 1970 are given. The operative mortality rate has been 5% and the late mortality rate 6%. Anticoagulants were sto...

Haemodynamic changes in patients with high pulmonary vascular resistance after mitral valve replacement. Comparative study between use of unstented aortic homograft valves and Starr-Edwards prostheses.

The clinical and haemodynamic responses to mitral valve replacement were studied in 36 patients with a pulmonary vascular resistance of4 units or above. In ig patients, I3 women and 6 men (mean age 49-7 years) Starr-Edwards prostheses were used. In I7 patients, 7 women and IO men (mean age 53 6 years) fresh unstented aortic homografts were inserted. In the latter group, 3 patients had, in addit...

Health-Related Quality of Life after Valve Replacement Surgery

Background: Heart valve disease gradually increases the pressure in the heart chambers and impairs heart pumping. It influences on quality of life due to low output, disability, fear, amnesia, anxiety, and depression. Although most of signs and symptoms in patients with heart valve diseases may significantly decrease after valve replacement surgery, there is limited information about the impact...
